Inspector Notes

A monitoring inspection was completed on 11/01/22. There were 9 children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records were reviewed. The inspection started at 10:30am and concluded at 11:30pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-330-B
Based on measurement, the center failed to maintain the required resilient surfacing depth and fall zone.
Evidence:
1. The mulch around the three slides and 2 climbers had mulch 2-3 inches in the fall zones. The required depth is 6 inches. 2. The red tricycle piece of equipment had approximately 4 feet of use fall zone around it, and the required fall zone is 6 feet. 3. The picnic table is in the fall zone of the slide.
Plan of Correction: Mulch will be added to the playground, and the 2 pieces of equipment moved to meet the requirement.

The center failed to have a working battery-operated radio .
Evidence:
1. There was not radio in the classroom.
