Inspector Notes

An unannounced renewal inspection started and ended on 1/31/24 from noon until 1:45. There were 10 children and 2 staff present.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 staff records and three children?s records were reviewed. Children were observed at lunch and during rest time.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the violation notice issued to the program.

Based on record review, the center did not complete all the required out of state background checks.
Evidence:
Staff #3 indicted on the sworn disclosure form they had lived out of state in tow states in the last 5 years and the criminal background checks had not been completed prior to being hired.
Plan of Correction: The necessary paperwork will be submitted immediately.

Based on record review, the center failed to have all the required information in the children's records.
Evidence:
Child #1 did not have two emergency addresses listed.
