During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 2/14/2025 and completed on 3/21/2025 in response to a self-reported incident received by the licensing office on 02/14/2025 relating to staffing and supervision. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 2/14/2025, child #1 (age 4 years, 2 months) was left unattended by center staff for undetermined time but possibly for between 3-6 minutes in the public school hallway. School personnel #1 found the child in the hallway outside the classroom. Center staff #1 was in the hallway and noticed child #1 with the school personnel and got the child and brought back to the classroom. Center staff #2 and #3 were on the bus when they realized child #1 was not on the bus and went back to the school to locate the child.
Plan of Correction: The Transportation Manager and Education Manager met and comprised the following plan for Riverview transitioning in the afternoon following the incident on February 14, 2025 which involved staff #1 and staff #2 which both employees have received a written violation in their personnel file. The transportation and education manager will monitor the transition plan for two weeks to ensure effectiveness. The Transition Plan for children released in the afternoon will be effective immediately, starting March 10, 2025. Please note: due to severe flooding immediately after the incident, the center was closed and then on a virtual schedule. Bus #31: Children will depart first. The part-time driver and part-time monitor will remain in the foyer as requested by the principal. Teachers from RV 1 & 2 will escort the children to the driver and monitor and do attendance/headcount while loading the bus. The driver and monitor will assist the children in buckling into their assigned seats and do an attendance/ headcount at the same time. When all children are secure, the monitor will note the time each child boards the bus.
Bus # 28 Children will depart second. The driver and monitor who are in the classrooms will, with the help of the teacher, escort their children to the bus and do attendance/ headcount while loading the bus. The driver and monitor will assist the children in buckling into their assigned seats and do an attendance/headcount at the same time. When all children are secure, the monitor will note the time each child boards the bus.
Bus # 35 Children will depart third. The driver and monitor who are in classrooms will, with the help of the teachers, escort their children to the bus and do attendance/ headcount while loading the bus. The driver and monitor will assist the children in buckling into their assigned seats and doing an attendance/ headcount simultaneously. When all children are secure, the monitor will note the time each child boards the bus.
When the driver arrives at the child's drop-off location, the monitor will record the time the child is released to the adult or emergency contact. When the bus run is completed, the monitor will write the total number of children released to the adults. When the bus arrives back at the school parking destination, the monitor and driver will do a bus sweep before exiting the bus for the day.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 02/14/2025, child #1 (age 4 years, 2 months) was left out of sight and sound supervision of center staff for an undetermined amount of time but possibly between 3-6 minutes.Child #1 was found by school personnel #1 in the hallway outside the classroom in the public school hallway. Center staff #1 was in the hallway and noticed child #1 with the school personnel and got the child and brought back to the classroom.
