Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on Thursday, October 6, 2022 from approximately 11:00am to 12:45pm. There 23 children in attendance and a total of four staff supervising. A total of five children?s files and five staff files were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the vendor did not maintain a written hard copy record of daily attendance that documented the arrival and departure of each child in care as it occurred, for each grouping of children.
Plan of Correction: We will create an attendance sheet for each classroom to track daily attendance.
Standard 8VAC20-790-540-B
Based on review of five children's records, the vendor did not ensure five children's records contained the required information. Evidence: 1. The records for Child #1 and Child #3 did not contain the address for the listed emergency contacts. 2. Staff #5 confirmed this information during the inspection.
Plan of Correction: We will add the address for the emergency contacts for the children enrolled and request the emergency contact addresses going forward.
