Inspector Notes

The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 25, 2023 from approximately 2:40 PM to 4:30 PM. There were 18 children in attendance and three staff member supervising. A total of five children?s records and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five children's records, the vendor did not ensure two children's records contained the required information.
Evidence:
1. The records for Child #1 and Child #3 did not contain the address for the listed emergency contacts.
2. A member of management confirmed this information during the inspection.
Plan of Correction: We will request the information and add it to the record.
Standard 8VAC20-790-550-2-a
Based on review of five staff records, observation and interview, the vendor failed to obtain satisfactory results of the Virginia Child Protective Services Central Registry check for onestaff member.
Evidence:
1. The record for Staff #1 (Date of Employment 10/25/2022) did not contain results of the Virginia Child Protective Services Central Registry check.
2. Staff #1 was observed working with children in the center during the inspection.
3. A letter dated 12/09/2022 indicated the information was not eligible for background portability.
4. A member of management stated there was confusion on the correct steps for obtaining the background check online and thought it was completed correctly,
Plan of Correction: The error was corrected while the licensing inspector was onsite.
