Standard 8VAC20-790-560-B
Based on review of two staff records and interview, the vendor did not ensure subsequent tuberculosis (TB)screenings were required at least every two years from the date of the initial screening for one staff member.
Evidence:
1. The record for Staff #1 contained a TB screening dated 07/21/2021.
2. Staff #1 confirmed an updated TB screening had not been obtained as of 10/25/2023.
Plan of Correction: I will tell the staff member the tuberculosis screening will need to be completed.
Standard 8VAC20-790-810-C-1
Based on observation, the vendor did not ensure when food is brought from home, the food containers were clearly labeled in a way that identifies the owner.

Evidence:
The licensing inspector observed lunch bags and lunches sitting on the table in the Preschool Classroom. Three of the six lunch bags did not contain a labeling system that identified the owner.
Plan of Correction: I will remind parents to label the children's lunches and we will check in the mornings as well.
