Inspector Notes

An unannounced monitoring inspection was conducted on 12/12/2023 from 10:20 a.m. to 1:45 p.m. There were 102 children present, ranging in ages from four months to five years old with 21 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation and interview, the center failed to complete out of state background checks for all staff who lived out of state within the past five years.
Evidence:
1. Staff #2 indicated on the sworn disclosure that they had lived in Maine within the past five years. Staff #10 confirmed that there were no out of state background checks for Maine for Staff #2 on file.
2. Staff #10 confirmed that Staff #2 at lived in New Hampshire within the past five years. Staff #10 confirmed that there was no abuse and neglect registry check and no sex offender registry check for New Hampshire for Staff #2.
