Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/17/2024 and completed on 12/19/2024. The on-site inspection began at 11:30 AM and ended at 2:15 PM. The inspector reviewed compliance in the areas listed above. There were 112 children present and 23 staff. The inspector reviewed 7 children?s records, and 7 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff #7, who had been employed for almost two months before the CPS was returned.
Plan of Correction: In the future all backgrounds will be followed up on within the 30 days and will be documented.
