Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 06/17/2025 and completed on 06/17/2025. The on-site inspection began at 10:20 AM and ended at 12:20 PM. The inspector reviewed compliance in the areas listed above. There were 128 children present and 24 staff. The inspector reviewed 7 children?s records, and 7 staff on-site. This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children and the documentation shall be signed by a physician, physician?s designee or an official of the local health department.

The screening for Staff #7 was not signed by a physician, physician?s designee or an official of the local health department.
Plan of Correction: The administration contacted the staff to ensure a signed copy of the TB is obtained.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The front panel of a cabinet was missing the corner piece and had sharp edges
Plan of Correction: The administration contacted the building and grounds to make them aware of the cabinet and will ensure that the front panel is replaced.
