Inspector Notes

A renewal inspection was conducted on 8/26/2022. There were a total of 45 children with 14 staff members present. Children were observed during nap time, and the infants were observed engaging in tummy time and floor play with their providers . The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of staff records, they did not have fingerprints on file prior to the employee starting.
Evidence:
Staff #1 (Date of hire 8/4/2022) had fingerprint results dated 8/11/2022.
Plan of Correction: We will get fingerprint results before letting employees begin work.
Standard 22.1-289.035-B-4
Based on a review of records, they did not have a sex offender check for any state the individual has resided in the preceding five years.
Evidence:
Staff #1 did not have a sex offender check on file for states previously resided in.
Plan of Correction: We ran the sex offender check on the DOJ website.
Standard 8VAC20-780-160-A
Based on a review of staff records, each staff member did not submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment.
Evidence:
Staff #2 (date of hire 7/18/2022) had a tuberculosis test dated 5/28/2022.
Plan of Correction: Employees will get their TB test within 30 days of being hired.

Based on a review of staff records, staff did not have documented orientation training within seven days of assuming job responsibilities.
Evidence:
Staff #3 did not have documentation of orientation in their file during the inspection.
Plan of Correction: We will document orientation when employees complete it.

Based on a review of staff records, documentation that two or more references as to character and reputation as well as competency were not checked
before employment.
Evidence:
3 of 3 staff files did not have references on file during the inspection.
