During the inspection, the inspector reviewed the areas listed above. The Department issued a Notice of Intent to place licensee on probation and require licensee to contact parents, guardians or other responsible persons regarding health and safety violations on 01/30/2025 as a result
of high-risk violations. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was completed on 07/16/2025, as a part of probation period. The on-site inspection began at 09:20 am and ended at 09:45 am. The inspector reviewed compliance in the areas listed above. The inspector reviewed 5 children?s records and 4 staff records on-site. This inspection included documentation review, and a tour of the infant classrooms.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
