During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/15/2025 and completed on 10/15/2025. The on-site inspection began at 12:10 pm and ended at 1:35 pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 15 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/25/2025. A POC submitted after this date will not appear on the public website.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Child #5, enrolled for 2 weeks, did not have an address listed for 2 of 2 people to call in an emergency.
Plan of Correction: Ensuring that all parents provide complete emergency contacts information for their children, including current phone number and physical address, on the emergency contact form.
Keeping a record of the communication with the parents that resisted providing their children emergency contact information.
