Inspector Notes
An unannounced monitoring visit and facility tour was conducted on 3/22/2023. At the time of inspectors' arrival there were 73 preschool aged children in care with 12 staff members and 2 case managers. Children were observed interacting with staff, engaged in outdoor play, educational activities, free play, lunch and hand washing. A sample of 5 children's records and 5 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

1. The fire inspection available was dated 3/2/22.
2. Staff confirmed that there was not an updated fire inspection available.
Evidence:
1. There was peeling paint throughout the center and on the playground equipment.
2. There were cracked floor tiles across the midline of multiple classrooms.
3. There were 10 broken floor tiles in Classroom C with exposed splintering subfloor.
4. There was peeling cove base in Classrooms E and F.
5. In Classroom F the large cabinet does not lock reliably.
Evidence:
1. There were cleaning chemicals in the large unlocked metal cabinet in Classroom E.
2. Staff stated that the cabinet lock does not work reliably.

2. Staff confirmed that an updated plan has been requested but was not available during this inspection.
