Inspector Notes

An unannounced renewal inspection was conducted on 04/25/2023 between the hours of approximately 9:30 a.m. and 10:45 a.m. There were 12 children present with the Provider and 2 assistants for a total of 25 points within the supervision guidelines. The children were observed cleaning up and preparing to go outside. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and caregiver records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.036-A
Based on record review and caregiver interview, the family day home did not obtain a copy of the results of a search of the child abuse and neglect registry for each employee every five years.

Evidence:
1. 3 out of 3 caregiver records reviewed, Caregiver #1, Caregiver #2 (start date: 10/20/2011) and Caregiver #3 (start date: 08/20/2015), do not contain current documentation of the results of a Central Registry search.
2. Caregiver #1's last central registry results was dated 02/06/2017.
3. Caregiver #2's last central registry results was dated 05/26/2016.
4. Caregiver #3's last central registry results was dated 06/07/2016.
5. Caregiver #2 stated that it was in process and they mailed off the requests for a central registry check for Caregiver #1, #2 and #3 a few weeks ago.
Plan of Correction: It is in progress. We mailed them off a few weeks ago.
Standard 8VAC20-800-210-A
Based on record review, not all caregivers obtained a minimum of sixteen clock hours of training annually.

Evidence:
1. Caregiver #1 obtained 9 clock hours of training for 2022-2023.
2. Caregiver #2 obtained 9 clock hours of training for 2022-2023.
3. Caregiver #3 obtained 2 clock hours of training for 2022-2023.
Plan of Correction: We will do 16 hours this year.
Standard 8VAC20-800-280
Based on observation, potentially poisonous substances were accessible to children.

Evidence: At the time of inspection, there was a spray bottle of all-purpose cleaner and a spray bottle of daily shower cleaner on the toilet in the bathroom that is used by the children.
