During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on March 13, 2025. The on-site inspection began at 11:40 a.m. and ended at 12:50 p.m. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 10. The inspector reviewed 5 children?s records and 4 caregiver records on-site. This inspection included document review, tour of the family day home and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 9, 2025. A POC submitted after this date will not appear on the public website.
Violations
6Child #3, #4, and #5 have been attending the family day home for nine months. The provider does not have documentation of a physical examination for Child #3, #4, and #5.
A disinfectant spray was stored under the changing table, accessible to children.
At the time of inspection, the sandbox was not covered and was not in use.
The records for Child #3, Child #4 and Child #5 did not contain the policy number of the child's medical insurance, proof of the child's age and identity, and written authorization for emergency medical care.
There was no documentation of emergency evacuation drills for December 2024 and February 2025.
Child #3, #4 and #5 have been enrolled at the family day home for nine months. The family day home does not have documentation of immunizations for Child #3, #4 and #5.
