During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/1/2026 and completed on 4/1/2026. The on-site inspection began at 1:40pm and ended at 3:10pm. The inspector reviewed compliance in the areas listed above. There were 22 children present and 6 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. The inspection included document review, tour of the facility, interviews and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #4 TB was dated 57 days prior to their date of employment.

**Repeat Violation**
Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

A can of disinfectant spray and disinfectant wipes with warning labels, ?Keep out of reach of children,? were unlocked in the infant classroom, out of reach of children.

Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.

Staff #4, who did not meet lead teacher qualifications, was alone with a group of preschool children for the duration of the inspection.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

During the inspection, eight preschool aged children were observed in a classroom unsupervised at rest time for approximately 3-4 minutes prior to staff #2 reportedly returning from break.

During the inspection, child #1 (7 months old) was left unsupervised in the infant classroom for approximately 2-3 minutes while staff #3 walked another child to the front of the center for dismissal. After, staff #3 walked into another classroom before returning to the infant room.

**Repeat Violation**
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for Child #1, enrolled for over three months, was missing the following required components: the name, address and phone number of one designated person to call in an emergency if a parent cannot be reached. The record for Child #3, enrolled for over five months, was missing the following required components: the name, address and phone number of one designated person to call in an emergency if a parent cannot be reached.
