Inspector Notes

An in person inspection was conducted today. All rooms are in ratio and resting during the inspection. The playground, emergency supplies, first aid supplies, emergency evacuation drills, shelter-in-place drills, infant bottles labeled/dated and medications are all in compliance today.
3 staff and 4 child records were reviewed. See Violation Notice for non-compliances.
LI was present today from 1:05 PM - 2:30 PM.

Standard 8VAC20-780-160-A
Based on a review of records, staff records did not contain documentation of a TB screening result within 30 days and prior to employment.
Evidence:
2 of 3 staff records reviewed contained documentation of TB screening results after the date of hire or more than 30days prior to employment.
* Staff 1; (Date of hire (DOH): 7/25/22.) TB results are dated 6/9/22 more than 30 days prior to employment,
*Staff 2 (DOH: 7/18/22) TB results are dated 7/22/22.
Plan of Correction: Moving forward, staff will be asked to obtain a note from their doctor is their results are more than 30 days old. All staff will provide documentation prior to employment.

Based on observation an infant was not placed in the crib on his back.
Evidence:
Staff was observed placing an infant in their crib on their stomach during today's inspection in the Infant 2 room.

Plan of Correction: Staff will be retrained and given the Bright Horizons policy for placing children in cribs again.
