Inspector Notes

An unannounced inspection was begun on 12/5/23 in response to a self reported incident being received in our office.
Based on multiple staff interviews, written statements and inspection of the playground, a violation for lack of supervision was issued.
2 staff records were reviewed. See Violation Notice for non-compliances cited.

Repeat Violation
Based on a review of records, one staff record did not contain documentation of a completed background search for an residence outside of the state of Virginia in the past 5 years.
Evidence:
1. There was no out of state Criminal History Search conducted prior to employment for a residence outside of Virginia in the past 5 years.
2. There was no Child Abuse and Neglect search results available within 45 days of employment for a residence outside of the state of Virginia in the past 5 years.

Plan of Correction: Admin will make sure to obtain all background checks prior to employee start or within 45 day timeline to obtain out of state background checks
Standard 8VAC20-780-340-F
Based on a multiple staff statements, one child under 10 years of age was not kept in actual sight and sound supervision.
Evidence:
1. On 12/4/23, Child #1 was not kept in actual sight and sound supervision when their class transitioned from the playground to the classroom.
2. The child was alone on the playground for approximately 1 -2 minutes.
Plan of Correction: Teachers will be retrained on our policies on children safety, supervision and transition tracking.
