Inspector Notes

An in person monitoring inspection was conducted today. All rooms were in ratio during the inspection.
Children were observed playing outside on the playground, eating breakfast, washing hands and playing with a variety of toys.
2 new staff records, 3 child records, medications, allergy action plans, emergency evacuation drills, first aid supplies, emergency supplies, infant bottle labeling/dating, playground resilient surfacing, outdoor toys and play equipment are all in compliance today.
The injury report form in use does not include all required components.

Based on a review of injury reports, reports do not contain all required components.
Evidence:
6 injury reports were reviewed. Missing documentation includes:
*6 of 6 reports reviewed were missing future action to prevent recurrence. The online form in use by the facility does not include this component.
*2 of 6 reviewed were missing signature of the parent or 2 staff signatures.
*2 of 6 were missing time of parent notification.
Plan of Correction: Director is working with regional manager to update occurrence report system. Until it is updated, staff will add necessary info to the forms.
