Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts , documents review, interviews, and a virtual tour of the program.

A monitoring inspection was initiated on July 21, 2021, and concluded on July 22, 2021. The director was contacted by telephone and a virtual inspection was conducted . There were 1O children present, ranging in ages from infants to 4 years old, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision,
programming , medication, special care and emergencies and nutrition . A total of 2 child records, 2 staff records, 2 household records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program .

Standard 22VAC40-111-180-A
Based on staff and household records review, the provider did not obtain documentation for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screen-ing or more frequently as recommended by a physician or the local health department.

Evidence:
Staff #1 Tuberculosis Screening expired on 1/05/2020.
Staff #2 Tuberculosis Screening expired on 8/16/2020.
Household member #1 Tuberculosis Screening expired on 1/17/2020.
Plan of Correction: I will get the Tuberculosis Screening done asap.
Standard 22VAC40-191-40-D-5-a
Based on household records review, the pro-vider did not obtain documentation that a person .14 years of age and older must have a search of the central registry and make the information available for regulatory purposes.

Evidence: Household member #2 did not have central registry results documentation in record available for review during the inspection.
Plan of Correction: I will get the central registry done for all household members asap.
