Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.

A monitoring inspection was initiated on 10/25/2021 and concluded on 11/4/2021 in response to a self-report that the department received on 10/20/2021 regarding forbidden action and behavior guidance. Interviews were conducted and documentation was reviewed. The inspector reviewed compliance in the areas of administration, staffing and supervision, staff qualifications and training, programming, and background checks.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the self-report is determined to be not valid. There are no violations resulting from this inspection.
