Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and remote inspection.

A complaint inspection was initiated on 1/12/2022 and concluded on 1/14/2022. A complaint was received by the department regarding allegations in the area of Special Care Provisions and Special Services. The director was contacted by telephone to conduct the investigation. A remote inspection was initiated on 1/12/2022 and again on 1/14/2022 to review the allegations. Documentation was reviewed related to the complaint.

The evidence gathered during the investigation did not support the allegations regarding preventing the spread of disease. There was no non-compliance with standards or law found during the investigation.
