Inspector Notes

An unannounced monitoring inspection was conducted from 9:50 am - 1:50 pm with the center director. There were 100 children, ranging from three months to four years of age, present with 20 staff supervising. Infant care was observed and older children were observed heading outside to play on the playground, having diaper changes, listening to stories, preparing for and eating lunch, and napping. Lunch served today: meatballs, carrots, pineapple and milk. Eight child records and six staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, five medications and authorization forms, three allergy care plans and the annual fire and health inspections. Based on an inspection of the playground, it was determined that the mulch under and around composite play equipment was not resilient. Evidence: The depth of the wood chips under and around the composite play structure on the preschool/school age playground was approximately one - three inches in depth.

Standard 8VAC20-780-500-A
(Repeat violation) Based on observation, the facility failed to ensure that children's hands were washed as required. Evidence: 1) In the infant classroom two infants were observed drinking bottles and their hands were not washed before and after as required. 2) In the toddler classroom, not all children had their hands washed after having a diaper change. 3) In the toddler classroom, staff were observed changing diapers and did not always wash their hands before and each child's diaper change.
