During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced investigation was initiated on 3/13/25 and completed on 3/25/25 in response to a complaint received by the licensing office on 3/11/25 relating to allegations of care, protection and guidance, ratios, and clean, safe, and operable. This inspection included document review, tour of the facility, interviews, observations, and measurements. The preponderance of evidence gathered during the investigation supports the allegations of clean, safe, and operable therefore, the complaint is determined to be valid.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/17/25. A POC submitted after this date will not appear on the public website.

Repeat Violation

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.

Evidence:

In the Make-Believe Blvd. room, a two-inch circular hole was observed on the wall leading to the loft portion of the play structure. A folding table was placed in front of the stairs but did not sufficiently prevent access to the hole or the broken rail that was laying at the top of the stairs. It was reported that the rail has been broken for two weeks.

In the Make-Believe Blvd. room, a bookshelf was partially blocking access to the slide exit of the loft area; evidence of children playing in that area was observed and included a pair of scissors, two large plastic connectors, a plastic pot and pan, and a connect four game.

In the ?Garage? play area of the Make-Believe Blvd. room, a large rectangular box measuring 46 inches x 18 inches was laying on the ground causing a trip hazard. Evidence of children playing around the box was observed and included a folding chair, home living vests, and ten large plastic connectors.

Children were observed in the Make-Believe Blvd. classroom at the time of investigation, and it is reported that the school agers use this room before and after school and on days that public school is out.
