During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 3/13/25 and completed on 3/25/25, in response to a self-report incident received by the licensing office on 3/13/25 relating to an injury requiring outside medical treatment. This inspection included document review, tour of the facility, interviews, observations, and measurements. Though not a part of the original self-report, violations that were not reported were found related to resilient surfacing and are documented on the violation notice issued to the program. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 4/17/25. A POC submitted after this date will not appear on the public website.

Repeat Violation

Areas and equipment of the center, inside and outside shall be maintained in a clean, safe, and operable condition.

Evidence: On March 13, 2025, a child was running on the older playground, tripped on a cement surface, fell forward, hitting the side of their face on the picnic bench, the impact caused bleeding near the eye. The gap between the resilient surfacing and the cement surface measured 1-1/2 inches causing the trip to occur.

Where playground equipment is provided, at least six inches of resilient surfacing shall be under equipment with moving parts or climbing apparatus to create a fall zone.

Evidence: On the older playground there was less than one inch of mulch at the base of the slide and the fall zones of the composite play structures.
