During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 5/27/25 and completed on 6/12/25 in response to a self-report received by the licensing office on 5/15/25 relating to behavior guidance. The inspector reviewed three staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/16/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-400-A
In order to promote the child?s physical, intellectual, emotional, and social well-being and growth, staff shall interact with the child and one another to provide needed help, comfort, support and promote ways of getting along.

Evidence: On 5/14/25 at approximately 3:20 p.m., during playground time, Staff #1 and Staff #2 from the Preppers class (two-years-old) and Staff #3 from the Preschool 1 class (three and four-years-old) became involved in a verbal argument. Staff #1, Staff #2, and Staff #3 used use profanity and argued in the presence of the children they were supervising and continued to argue when administration had arrived on the playground to de-escalate the situation. Staff #2 was heard saying, to Staff #3 ?Run up then, run up and do something.? Staff #3 was then observed grabbing a bottled beverage from the ledge of the building and approaching Staff #2 on the Prepper?s playground, Staff #2 was observed throwing their drink over the fence and into the face of Staff #3. The argument lasted approximately three minutes.
Plan of Correction: Removed all staff involved immediately

Retrained staff on proper interactions with others and while in the presence of children.
