Where playground equipment is provided, at least six inches of resilient surfacing shall be under equipment with moving parts or climbing apparatus to create fall zone.

Evidence: On the older playground there was two inches of mulch in front and on all sides of the slide and fall zones of the composite play structure. The mulch has deteriorated into compacted dirt and is no longer resilient. Six inches of resilient surfacing is required in the fall zones of the composite play structure including slide exits and stairs.
Plan of Correction: The center immediately contacted our landscaping company. The landscaping team responded and completed the corrective work the very next day. Six (6) inches of resilient surfacing was added and evenly distributed throughout all required fall zones on the older playground, including under and around the composite play structure, slide exits, and stairs to be in full compliance.
To prevent reoccurrence, playground surfacing depth will be monitored regularly
as part of routine playground safety checks. Additional mulch will be added as needed to ensure that the required six (6) inches of resilient surfacing is consistently maintained in all fall zones.
