Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A renewal inspection was initiated on 10/14/2021 and concluded on 10/15/2021. The Provider was contacted by telephone and a virtual inspection was conducted. There were 5 children present.. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 child records and 1 caregiver record and 1 household member record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of documentation and interview, the provider has not obtained a physical exam for each child within 30 days of enrollment.
Evidence:

Based on interview, the provider has not obtained a negative tuberculosis (TB) screening for each adult household member.
Evidence: Household member #2 turned 18 years old in March of 2020. The provider has confirmed that household member #2 has not received a TB screening. This was previously cited in September 2020.
