Inspector Notes

An unannounced renewal inspection was conducted today between 10:30am and 11:30am. There were 3 children present with the provider for a total of 6 points within the supervision guidelines. The children were observed playing freely. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-800-100-A
Based on record review, the provider did not obtain documentation of a physical exam for each child within the first 30 days of attendance.
Evidence: Child #1 (date of enrollment: 4/20/2022) file does not contain documentation of a physical exam.
Plan of Correction: I'll call the parents and request that they bring me a copy.
Standard 8VAC20-800-50-A
Based on record review the family day home does not keep a written record of children in attendance each day.
Evidence: The provider did not have any documentation of attendance taken prior to the current week. This was previously cited on 6/30/2022.
Plan of Correction: I'll leave it out so I remember to do it everyday and I'll keep the copies.
Standard 8VAC20-800-770
Based on observation, the provider does not have a working battery-operated weather band radio.
Evidence: The provider did not have the correct size batteries for the radio to work.
Plan of Correction: I'll get it working today.

Based on record review and interview, the provider did not obtain documentation of immunizations for each child prior to the first date of enrollment.
Evidence: Child #1 record does not contain documentation of immunizations.
