Inspector Notes

Licensing Inspector conducted an unannounced Monitor Subsidy Inspection on December 1,, 2022. There were no children present during the inspection. Currently no children enrolled in the after school program therefore the Licensing Inspector did not review any children records. The Licensing Inspector reviewed 1 staff record along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review & discussion with the Center Director, the Vendor failed to ensure staff who work directly with children annually attend required training and staff development to include the department's 3 hour health and safety update course.

EVIDENCE:
The 3 hour annual health and safety update course has not been completed by staff for year 2021 & 2022.
Plan of Correction: Staff will complete 3 hour annual update training. Documentation of completed training will be maintained in the staff record.
