A monitoring inspection was initiated on 5/01/2024 in response to a Injury Requiring Outside Medical Treatment report.
Staff interviews were conducted, outside play was observed, and documentation was reviewed.
Non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

2Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. On 4/26/2024, there were approximately 23 preschool aged children on the playground with three staff supervising. According to interviews and written statements, three staff were reportedly grouped together, checking the attendance logs on the ipads, under the shade covering. Child 1 sustained a significant injury to their arm, which resulted in emergency surgery. None of the three staff present saw or knew the cause of the injury.
Plan of Correction: The school has a posted playground position checklist on the playground. This checklist has pictures and written instructions outlining the expectations of teacher positioning while supervising children during outdoor play. All staff involved have been retrained on playground safety and supervision. The teachers were also retrained on maintaining supervision during attendance taking procedures for children that are picked up during outdoor play, by assigning one teacher at a time to be responsible for this task

The center shall develop written playground safety procedures which shall include a policy on active supervision on the playground. Evidence: On 4/24/2024, there were 23 children on the playground. Interviews revealed that staff on the playground were grouped together, checking attendance, and did not position themselves in strategic locations to ensure active supervision. Child 1 sustained a serious injury to their arm which resulted in emergency surgery.
Plan of Correction: The school has playground safety procedures in place. All staff were retrained on playground safety and supervision, and to specifically address staff supervision placement. All staff were provided a copy of the playground position and supervision expectations checklist. The school has a posted playground position checklist on the playgrounds with pictures and written instructions outlining the expectations of teacher positioning while supervising children during outdoor play.
