During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on January 15, 2025and the inspection concluded on January 16, 2025. The on-site inspection began at 12:30 p.m. and ended at 4:30 p.m. The inspector reviewed compliance in the areas listed above. There were 107 children present and 16 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. Additional documents were submitted electronically on January 16, 2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-A
The center is required to have employees undergo a background check and a search of the central registry (CPS check) every five years. The most recent central registry check for Staff #9 was overdue 7 months and the national fingerprint check was overdue by 8 months.
Plan of Correction: The CPS check request has been submitted. An appointment for fingerprints will be as soon as possible.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. Staff 7's updated screening was due 1 month ago and Staff 9's updated screening was due 8 months ago. The updated screenings have not yet been completed.
Plan of Correction: Staff will get updated TB tests or screenings as soon as they can be scheduled.

The center shall provide to the licensing representative an annual fire inspection
report from the appropriate fire official having jurisdiction. The last documented fire inspection was conducted 7/26/2023. A fire inspection was conducted following the licensing inspection on 1/16/2025
Plan of Correction: The fire inspection was conducted 1/16/2025.
