During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on June 16, 2025. The on-site inspection began at 11:15 a.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 117 children present and 17 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Documentation of a physical examination must be submitted for each child within 30 days after the first day of attendance. Child 8, enrolled for over 4 months, did not have documentation of a physical.
Plan of Correction: Physicals will be obtained within 30 days of attendance, as required.
Standard 8VAC20-780-245-J
Whenever the center has agreed to administer prescribed medications, the administration shall be performed by a staff member who has satisfactorily completed a training program for this purpose approved by the Board of Nursing Staff (Medication Administration Training training). Neither of the 2 staff with MAT certification were on site today from approximately 9:00 a.m. and 12 p.m. - 5 children on site have emergency medications.
Plan of Correction: Additional staff will obtain MAT training.
