An unannounced monitoring inspection was initiated on June 16, 2025 and completed on July 15, 2025 in response to a self-report received by the licensing office on June 9, 2025 relating to termination policy. The inspector reviewed 2 children?s records on-site. This inspection included document review, tour of the facility, and interviews.
