During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 2/18/2026 and completed on 3/16/2026 in response to a self-report received by the licensing office on 2/12/2026 relating to forbidden actions. The inspector reviewed 1 child?s record and 3 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/24/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On February 12, 2026, Staff #1 grabbed child #1 (age 2) by the arm and threw them onto the floor in the front lobby of the center with 6 other children present. Child #1 reported their right elbow hurt and was crying immediately after this incident.
Plan of Correction: Staff ensured care, protection and guidance of all children during the incident by responding immediately and appropriately. A manager attended to child #1 without delay, while another staff member safely escorted the remaining children to the classroom t maintain a secure and supervised environment. Prompt removal of staff member #1 further ensured the children's safety, and all subsequent actions, including timely reporting and parent notification, reflected a continued commitment to the well-being and protection of every child in care.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities and services are maintained in compliance with the center's own policies and procedures that are required by the standards.

The center's discipline policy states: ?There will be no physical punishment or disciplinary action administered to the body such as, but not limited to: roughly handling a child.?

On February 12, 2026, staff #1 roughly handled child #1 (age 2) by grabbing the child by the arm and throwing them onto the floor.
Plan of Correction: Staff member #1 acted in direct violation of company standards and was promptly removed from the premises and subsequently terminated. The manager then ensured that all actions taken following the incident were consistent with the center's established policies and procedures, to include immediately attending to child #1, while another staff member escorted the remaining children to the classroom, demonstrating proper supervision and safety protocols. The school director reported the incident to licensing authorities and Child Protective Services (CPS) without delay. Parents were immediately notified and informed of the incident and the procedures followed. Throughout the process, the center maintained full transparency and compliance with all applicable standards.
Standard 8VAC20-780-410-1
The following acts are forbidden: roughly handling a child.

On February 12, 2026, staff #1 grabbed child #1 (age 2) by the arm and threw them onto the floor in the front lobby of the center with 6 other children present. Two staff witnessed child #1?s feet off the ground at the time they were thrown to the ground.
Plan of Correction: At the time of the incident, a manager immediately attended to child #1, while another staff member escorted the six remaining children to the classroom. Staff member #1 was removed from the premises within minutes of the incident. The school director promptly reported the matter to licensing authorities and CPS. Parents were notified immediately and informed of the incident, as well as the procedures and protocols followed. The school maintained full transparency throughout its response and reporting process.
