During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/23/2024 and completed on 10/23/24, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 11:50am. The inspector reviewed compliance in the areas listed above. There were 114 children present and 21 staff. The inspector reviewed 10 children?s records, 5 staff records and 1 independent contractor record on-site. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/30/2024. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #9, who had been enrolled for over two months.
Plan of Correction: Parent was called to obtain those records by the physician.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #3's updated screening was due over 3 months ago. The updated screenings has not yet been completed.
Plan of Correction: Staff #3 was made aware to update TB screening and an appointment was scheduled.
Standard 8VAC20-780-230-B
Individuals from independent contractors who do not meet staff qualifications shall, when in the presence of children, be within sight and sound supervision of a staff member.

Independent contractor #1 was left alone without sight and sound supervision of a staff member in the Enrichment Room, where six children were practicing dance for approximately 30 minutes.
Plan of Correction: Staff employee will be with an independent contractor during enrichment classes.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

In the enrichment room, a one-and-a-half inch dented metal frame was exposed on the lower wall where children were having dance practice. In the Explorer-C Twos classroom, there were three areas of peeling paint, each measuring within 1 to 3 inches in length, within reach of children.
Plan of Correction: Order was placed to repair the dent. The dents was replaced with a one-and-a half inch rubber siding.

Hazardous substances must be in a locked place that prevents access by children.

In the Infant, Pre-K A, and Young 4's classrooms, cleaning solutions and sanitizing solutions were in an unlocked cabinet, out of reach of children.
Plan of Correction: Health and Safety training (repeat) to obtain safety regulations.

Children under 10 years of age shall always be within actual sight and sound supervision of staff.

During a 30-minute period, six children aged three and above were not within actual sight and sound supervision of staff while participating in dance lessons in the Enrichment Room with Independent Contractor #1.
Plan of Correction: Staff employee will be present with independent contractor to maintain all children are in sight and sound.
Standard 8VAC20-780-350-B-4
The required ratio for children ages 3 years to school age eligible is one staff per ten children.

Independent Contractor #1 was supervising dance lessons with six children aged 3 and above for approximately 30 minutes without a qualified staff member present to meet the required staff -to-child ratio.
Plan of Correction: Staff employee will be with an independent contractor during enrichment classes.
