REPEAT VIOLATION:
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
The green and brown chairs in the two to three year old classroom contained two tears in the seating material. The paint was peeling on the wooden window ledges in the toddler classroom and in the two-year old classroom.

Plan of Correction: The soft green chair and brown soft brown chair were removed from the classroom. The interior painting has a scheduled date for April for the entire school.

Plan of Correction: The Lego bin was immediately removed from the diapering surface in the toddler classroom. Retraining on proper diapering and diaper-changing procedures is in place for all staff to ensure compliance.

Plan of Correction: The missing date for Staff D's reference check have been documented. Administration staff will ensure that all reference checks, including those conducted by phone, include complete documentation with date of contact recorded. Administration staff will review personnel files prior, to employment.
