Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 4/22/2021 and concluded on 4/28/2021. The director was contacted by telephone to initiate the inspection. There were seven children present and two staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed two children's records and two staff records submitted by the facility to ensure documentation was complete.

The information gathered during the inspection determined that there were violations with applicable standards or law. Violations were cited on the Violation Notice and discussed with the center director.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review and discussion the center failed to ensure each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.
