Inspector Notes

The Licensing Inspector initiated and completed a monitoring inspection on 10/4/2022. The inspector was on site from 2:35pm until 4:20pm.

There were twenty- six children under the care of three staff. The ages of the children ranged from five to ten years old. The children were observed during outside time, snack time and homework time. Three children?s record and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Based on record review, the center failed to ensure each staff member has a documented TB test or screening every two years.

Evidence: Staff #2 was working day of inspection with an expired Tb test or screening. Staff #2 last TB screening was dated 09/20/2020
Plan of Correction: Staff member will be sent for a new test or screening.
