Inspector Notes
An investigation was initiated on 7/18/22 in response to a self reported incident on that same date regarding supervision of children. Documentation related to the incident was reviewed and interviews with collaterals and staff were conducted by telephone. An unannounced on-site inspection was conducted on 8/28/22 and additional interviews were conducted. Based on information gathered during the investigation, the self reported incident is determined to be not valid. No violations were issued.
