Inspector Notes
An investigation was initiated on 8/9/22 in response to a self reported incident on that same date regarding staff/child interactions. Documentation related to the incident was reviewed and interviews with collaterals and staff were conducted by telephone. An unannounced on-site inspection was conducted on 8/28/22 and additional interviews were conducted. Based on information gathered during the investigation, there was insufficient evidence to make a determination of valid. However, during the on-site inspection, violations not related to the self-reported incident were found. Violations are cited on the violation notice and were discussed with center management at the conclusion of the investigation.
Violations
3Evidence:
1. During an on-site inspection at the center on 9/28/22, the licensing inspector and staff 1 (center director) observed child 1, age 3 years, walking down the hallway alone and unsupervised up to the front of the center, while the rest of the child's class was outside on the playground. Child 1 was out of sight and sound supervision of staff for approximately 30 seconds to 1 minute.
2. Staff 2 and staff 3 both stated that they were not aware that child 1 had left the playground and gone inside the building.
3. Staff 3 stated that child 1 had attempted to go inside the center from the playground previously that morning and that the child "must have wandered back inside".
4. Staff 1 (center director) confirmed that child 1 was without sight and sound supervision of staff and had left the playground without staff knowledge.
Evidence:
1. During an on-site inspection on 9/28/22, the licensing inspector observed 1 staff person caring for 5 infants, ages 4 months to 9 months, in the infant room. The number and ages of the children required 2 staff to be present.
2. Staff 1 (center director) confirmed that there was only 1 staff person with 5 children in the infant room, where 2 staff are required.
Evidence:
1. The record for child 1 (date of enrollment: 9/6/22) does not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached.
2. Staff 1 (center director) confirmed that the record for child 1 is lacking the required emergency contact information.
