Inspector Notes

An unannounced renewal inspection was conducted on April 21, 2023. The inspector arrived at 1:30pm AM and departed at approximately 3:30pm. There were 51 children present and 12 staff members. A sample size of 10 records were reviewed which included five children?s records and five staff records. The Inspector observed staff and children?s interactions throughout the inspection. Children were observed during group activity, free play both indoor and outdoor, bottle feeding, and naptime. First aid kits and emergency preparedness documents were observed. Medications were observed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the site director at the exit interview.

Standard 8VAC20-780-440-N
Based on observation and interview, it was determined that the center did not ensure that toys or objects hung over an infant in a crib and crib gyms that are strung across the crib may not be used for infants over five months of age.
Evidence:
1. Three of the four cribs used by Child #1, Child #2, and Child #3 had toy objects that strung across the crib. All three children were over the age of five months.
a. Child #1 is age 10 months.
b. Child #2 is age 9 months.
c. Child #3 is age 19 months.
2. Infant staff member confirmed that Child #1, Child #2, and Child #3 had toys strung across their cribs and that each child was over the age of five months old.
Plan of Correction: The center responded with the following: The infant teacher immediately removed all toy objects from each crib during the course of the inspection. Moving forward, the toys strung over the cribs will not be used for children over the age of five months.
Standard 8VAC20-780-570-E
Based on observation and interview, the center did not ensure that prepared infant formula shall be dated.

Evidence:
1. In the infant room, there were 3 bottles of formula that were not dated.
2. The infant teacher confirmed that the bottled formula in the infant room were not dated.
Plan of Correction: The center responded with the following: The center will notify all parents face to face and via the brightwheel app that all bottles that come pre-made to the center shall be named and dated. The center will ensure that each bottle used in the infant room have both the name and date.
