Inspector Notes

An unannounced inspection was initiated on November 26, 2024 and completed on March 7, 2025 in response to a complaint received by the licensing office on October 29, 2024 relating to allegation of ratio, supervision, physical plant, nutrition and food services and hand washing and toileting procedures. The inspector reviewed 8 children?s records and 7 staff records on-site on December 20, 2024. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation of supervision, ratio, nutrition and food services and handwashing and toileting procedures therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (3/31/2025). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. The children in the waddler classroom were eating lunch in a dark classroom and there were four out of six seats on the feeding table that had broken straps.
Plan of Correction: Make sure lights are on and we have ordered new seat belts for the table.
Standard 8VAC20-780-290-A-2
Fans, when used, shall be out of reach of children and cords shall be secured so as not to create a tripping hazard. The waddler room had a fan cord that was plugged in and not secured on the side of the wall within reach of children.
Plan of Correction: Fan was removed from the room.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom provided that staff may need to provide intermittent sight supervision to provide assistance to children as needed. A child (age 3 years) was using the urinal, and two other children (ages 3 years) began to use the same urinal at the same time and a staff intervened about a minute later.
Plan of Correction: Children will use the bathroom one at a time with teacher standing outside of the door.

The staff-to-children ratios for children 3 years old to school age eligible is 1:10 and is required whenever children are in care. Staff #1 was alone in the classroom supervising 15 children aged three and four for approximately 3 minutes before another staff member arrived.
Plan of Correction: Inform staff about the requirements and make sure they call upon management for help.

Children's hands shall be washed with soap and running water after toileting. Two children in the three- and four-year-old classroom did not wash their hands after using the restroom.
Plan of Correction: Train staff on how to make sure the kids remember to wash their hands.

Disposable diapers shall be disposed in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. A staff member in the waddler classroom was observed touching the trash can with their hands to dispose of a diaper.
Plan of Correction: Remind teachers on proper diapering and to always use the foot pedal.
Standard 8VAC20-780-560-J
Tables shall be sanitized before and after each use for feeding. Staff in the two- and three-year-old classroom did not sanitize the tables before the children sat down for lunch.
Plan of Correction: Remind teachers to clean tables before and after.
