A subsidy inspection was initiated on May 16, 2025 and completed on May 16, 2025. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

For each group of children, the vendor shall maintain a written hard copy record of daily
attendance that documents the arrival and departure of each child in care as it occurs. There were 25 children present at the center and only 7 children were checked in on the attendance system. There was no observed written hard copy of daily attendance of the children in care.

Plan of Correction: A hard copy of the attendance sheet is now being maintained and updated in real time to document all arrivals and departures of children. Staff have been retrained on the importance of daily attendance recording and are required to complete both digital and hard copy check ins. The assistant director will verify morning attendance by 9:30AM and perform random checks throughout the day to ensure accuracy.

REPEAT VIOLATION
Within the first 90 days of employment or subsidy vendor approval all staff who work
directly with children shall complete Virginia Preservice Training for Child Care Staff. Of the 8 staff files reviewed, 4 staff member did not have documentation of completion of this training.

Plan of Correction: All non-compliant staff were removed from child supervision immediately until valid proof of completion pre-service training is submitted. Those still in need of training have been enrolled to complete the training immediately.
