Inspector Notes

An unannounced inspection was initiated on May 16, 2025 and completed on July 18, 2025 in response to a complaint received by the licensing office on April 2, 2025, relating to allegations of supervision and forbidden actions. The inspector reviewed 13 children?s records and 10 staff records on site. This inspection included document review, tour of the facility, interviews and observations. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/2025. A POC submitted after this date will not appear on the public website.

When staff are supervising children, they shall always ensure their care and guidance. On April 2, 2025 at approximately 9:00am, staff #1 was in the pre-k classroom sitting and looking at their cell phone for approximately five minutes and in that time, child #1, child #3, child #4 and child #5 (between the ages of 4-5) engaged in inappropriate actions with each other. Staff #1 did not notice the incident occurring. When Staff #2 entered the classroom, they immediately intervened with the group of children to end the inappropriate activity.

Children under 10 years of age always shall be within actual sight and sound supervision of staff. The infant room and the pre-k room each had one staff supervising the classrooms and on multiple occasions, the staff walked out of the classroom leaving the children unsupervised. Child #6 (age 5 years) was sitting in a chair unsupervised outside of the pre-k classroom for approximately two hours.

Enclosure in a small, confined space or any space that the child cannot freely exit himself is a forbidden action. Staff #3 placed child #2 (age 2 ? years old) in an infant crib in a separate area away from other children because they were trying to run out of the building.
