Inspector Notes

An unannounced monitoring inspection was conducted today from 9:15am-11:15am. There were 64 children directly supervised by 13 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 6 children?s records, 6 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in water play outside, making Father's Day crafts, participating in circle time, singing songs, and playing in centers. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. The center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the center did not obtain a physical examination signed by a physician for each enrolled child prior to child's attendance or within 30 days after the first day of attendance.

Evidence: Child #2 (First Day of Attendance: 8/29/2018) did not have a physical examination on file.
Plan of Correction: Current physical examination was placed in child's file on day of visit.

Based on review of medications, the center did not ensure that prescription medications are maintained in the original, labeled contained with the pharmacy issued prescription label attached.

Evidence: Child #7 had prescription medication on site that was not maintained with the pharmacy issued prescription label attached.
Plan of Correction: Medication was in original container. Paperwork was correct to reflect the instruction of the medication. Pharmacy issued label was attached on the day of visit.
