Inspector Notes
An unannounced monitoring inspection was conducted on 6/21/23 from 10:30 a.m. ? 2:00 p.m. There were one hundred and seven children present with seventeen staff providing supervision. The ages of the children ranged from 2 months to 12 years. Five staff files and five children?s files were reviewed. Children were observed in centers and free play. Lunch was cheeseburger, mixed vegetables, pears and milk. Handwashing was also observed. Attendance, menus, and emergency drills were reviewed. There was an adequate number of staff present with certification in Medication Administration Training (MAT), CPR, First Aid and Daily Health Observation (DHO) training. Six medications and their corresponding authorizations were reviewed. Required postings were observed. The last emergency drill was conducted on 6/16/23. The first aid kit, flashlight, and battery operated radio were observed. Areas of non-compliance are identified in the Violation Notice.

11Evidence: The record for Staff #5, with a start date of 3/2/23, did not contain documentation of the results of a search of a central registry search at the time of inspection.
Evidence:The record for Child #5, with a start date of 8/8/20, had a physical dated 6/15/22.
Evidence:
1. In the Infant A class, Child #2 had a crib sheet that was loose and did not fit the mattress tightly.
2. In Prep Jr. K class, a piece of yarn was tied in a half loop hanging down from the shelves in the restroom enclosure. The cord was hanging low enough to be within reach of the children, causing a strangulation hazard.
3. In the Montessori A class a carpet, under the sensory table, at the entrance of the restroom curled up and was folded on top of itself creating a trip hazard.
4. In the Stepping Stones class a carpet under the sensory table was curling at the corners creating a trip hazard.
Evidence:
1. In the School Age class, there were nine bottles of essential oils on the table with labels stating, keep out of reach of children.
2. The kitchen, located on the perimeter of the school age class, was left open during lunch service; several large containers of cleaning solutions were accessible to children.
Evidence:
1. In the toddler class, after diapering a child, staff #5 cleaned the surface, applied the disinfectant solution and immediately wiped it off, not allowing the surface to air dry as required.
2. In the Preschool Explorers room, the restroom contained a small foot operated trash can with no lid, filled to the top with soiled diapers.
Evidence: The center did not have a battery operated flashlight on site.
Evidence: In the School Age class there were two lunch boxes with no name and date and in the Montessori class there two lunch boxes that had no name and date.
Evidence:In the Infant A class, there was a bottle in the refrigerator that was not labeled with the child?s name and dated.
Evidence: The record for Child # 2, did not contain documentation that the center had viewed proof of the child?s identity and age.
Evidence:
1. The record for Staff #3 did not contain documentation of an emergency contact.
2.The record for Staff #2, with a start date of 11/10/22, contained documentation of one reference.
3.The record for Staff #5, with a start date of 3/2/23 did not contain documentation of two references.
Evidence:
1. In the Infant A and B class there were eleven children present in the classroom, the written record reflected there were nine.
2. In the toddler class there were ten children present in the classroom, the written record reflected there were nine.
