Inspector Notes
An unannounced investigation was initiated on 11/20/25 and completed on 11/20/25 in response to a complaint received by the licensing office on 11/16/25 relating to allegations of care, protection, and guidance. The inspector reviewed two children?s records onsite, and one staff record electronically on 11/21/25. This inspection included document review, tour of the facility, interviews, observations, and measurements
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
