Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on 1/21/22. The provider was contacted by telephone and a virtual inspection was conducted. There were 7 children present, with 2 caregivers supervising. The children were observed playing with toys, eating afternoon snack which included cheese quesadillas and oranges and milk. The outdoor space and play equipment used for children was reviewed. There were no medications on site. A total of 2 children's records, 2 caregiver's records and three household member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. For any questions regarding this inspection, please contact me at mahrukh.aziz@doe.virginia.gov or 571-835-4718.

Violations

1
Standard 22.1-289.035-A
Based on record review and staff interview, the family day home provider did not obtain results of a finger print based criminal history check for an employee prior to the first day of employment.
Evidence:
1. Caregiver # 1's first day of employment was 8/23/21.
2. The provider stated that Caregiver # 1 had completed the fingerprint process in July 2021 and she never got the results. The provider further stated that after following up with the agency and still not being able to obtain results, she hired Caregiver # 1 who started employment on 8/23/21.
3. Caregiver # 1 went back to get the fingerprint process completed and the fingerprint results were dated 1/6/22.
Plan of Correction: The reason I hired her was because I need to continue working and she was the right person to work in my place. Since I had made calls and sent emails saying that I need the results and the fingerprint office told me that they would send them to me and that never happened.
