Inspector Notes

A monitoring inspection was conducted on 8/31/23 from 10:15 am to 11:25 am. There were a total of 7 daycare children and 2 resident children (17 points total) present, with 2 caregivers supervising. The children were observed finishing up morning snack and playing with Legos and other toys. 4 household member records, 2 caregiver's records, 3 children's records, attendance, drill records and the emergency plan was reviewed.

Based on staff interview, the provider did not obtain a Sword Disclosure Statement (SDS) for each adult household member.
Evidence:
1. Household Member # 1 reached the age of 18 years a few months ago.
2. The provider stated that Household Member # 1did not complete a SDS to this date.
Plan of Correction: Household Member # 1 will complete the form and the inspector will be notified.
